Here are the application forms that you need to either put money in a new ISA or transfer your current one to a new provider.
Please make sure you have read:-

Form 1 - Investment ISA (stocks and shares) 2010/2011 Tax Year Form - Application Forms
• Please complete this form to invest in your ISA • If investing a lump sum attach a cheque payable to Cofunds Ltd
• If paying a monthly contribution, complete the direct debit form
• Enter the funds that you wish to invest in and the amounts. You can choose more than one fund
• Please check that ALL your details are entered, the form is signed and returned to us using our freepost address - see below
Form 2 - Investment ISA (stocks and shares) Transfer -
Application Forms
• Fill this form in to transfer your cash ISA or investment ISA to Cofunds Stocks & Shares ISA
• If transferring cash ISA’s please ensure you put your account number on the form
• If transferring investment funds please ensure you list all the funds & the account numbers
• If transferring more than one company’s funds, please copy the transfer authority form
• Please check that All your details are entered, the form is signed and returned to using our Freepost address below.
